We are seeking a dedicated Family Nurse Practitioner or Physician Assistant to join our innovative primary care team at One Medical in New York, NY. As a key member of our healthcare family, you will be responsible for managing a diverse patient panel, conducting in-office and virtual visits, and collaborating with interdisciplinary teams, all within a cutting-edge medical environment.

Responsibilities

• Manage a varied patient panel with acute, chronic, and well visit needs.
• Deliver exceptional in-office and tele-health care services.
• Participate in continuous learning opportunities and clinical rounds.
• Collaborate effectively with in-office and virtual clinical teams.
• Utilize clinical training to perform in-office procedures.

Requirements

• Completion of an accredited FNP or PA program with national certification.
• Minimum of 2 years as an Advanced Practitioner in outpatient primary care within the last 5 years.
• New York state licensure required by One Medical start date.
• Demonstrated human-centered primary care approach and proficiency in evidence-based practice.
• Strong communication and technology skills with a commitment to quality care.
