Home Care LPN - Available for Per diem, Part Time, Full Time

Job Description

We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. You will be responsible for implementing patient-centered care plans and participating in discharge planning. Our supportive leadership team and state-of-the-art Electronic Health Record system will help you excel in your career.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Implement patient-centered care plans and participate in discharge planning.
• Support New Castle County, DE; work day shift or night shift, one weekend a month, and 2 holidays a year.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
