Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. Our organization provides top-notch OASIS-E training and utilizes a cutting-edge Electronic Health Record system to support our staff.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in coordination with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
