Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare. As part of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. You will play a vital role in implementing patient-centered care plans and contributing to patient discharge plans. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
- Experience in HomeHealth or related areas is preferred.
