Job Description
We are looking for a dedicated Home Care LPN to join ChristianaCare's HomeHealth team. As an essential member of our organization, you will provide skilled nursing care to geriatric patients in the community and ensure the implementation of patient-centered care plans.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Execute goal-oriented care plans in alignment with patient needs and available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Compensation
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
