Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As a crucial member of our organization, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in accordance with available resources.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
Job Types
Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
