Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in collaboration with the healthcare team.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
Job Details
- Job Type: Full-time, Part-time, Per diem
- Pay: $22.00 - $28.00 per hour
- Expected Hours: 20 – 40 per week
- Weekly Schedule: 5x8, Monday to Friday, Rotating weekends
- Work Location: In-person
