We are looking for a skilled Home Care LPN to join our team at ChristianaCare HomeHealth. As an LPN, you will provide skilled nursing care and treatment to geriatric patients, help implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must have a current DE LPN license or compact state license.
