Job Description
ChristianaCare HomeHealth is looking for a dedicated Home Care LPN to join our team. As a Full Time, Part Time, or Per Diem LPN, you will provide skilled nursing care and treatments to geriatric patients in the community. You will work closely with patients, implement care plans, and actively participate in discharge planning. This role offers competitive pay ranging from $22 to $28 per hour.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
