Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare's HomeHealth division. Our organization is equipped with state-of-the-art technology and offers comprehensive OASIS-E training to support your career growth. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Weekly Schedule: 5x8, Monday to Friday, Rotating weekends.
