Job Description
ChristianaCare HomeHealth is seeking experienced Home Care LPNs to join our team in providing exceptional in-home nursing care to patients in New Castle County, DE. As an LPN, you will be responsible for administering skilled nursing care, implementing care plans, and facilitating patient discharge in a community setting.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in the development of discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
