As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and reimbursement.

Position Highlights

• Direct and oversee the RAI process, ensuring accurate completion, encoding, and transmission of MDS.
• Contribute to developing individualized resident Care Plans while adhering to Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare settings such as hospitals or long-term care facilities.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications, including windows programs.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification required.
