As an LPN, you will play a crucial role in the Resident Assessment Process by completing relevant items on the Minimum Data Set (MDS) to ensure appropriate patient care and nursing center reimbursement. Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of the MDS.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and electronic MDS completion to interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility is required.
• Proficiency in general nursing practices, medical procedures, and long-term care regulations.
• Computer skills including familiarity with Windows applications.
• AccuMed experience is a plus.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification is necessary.
