Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) to join Genesis HealthCare as an LPN MDS Coordinator. In this role, you will play a vital part in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure excellent patient care and nursing center reimbursement. As an LPN MDS Coordinator, you will direct the interdisciplinary Resident Assessment Instrument (RAI) process, develop individualized resident Care Plans, and train new team members.

Position Highlights

• Direct the RAI process and ensure accurate completion of MDS.
• Develop individualized Care Plans and identify resident needs.
• Train new team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare facility.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Computer proficiency, including windows applications.
• AccuMed experience preferred.
• Prior MDS Coordinator experience a plus.
• BLS/CPR certification required.
