Job Description

We are looking for a Licensed Practical Nurse (LPN) to join Genesis HealthCare, a leading healthcare services provider. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs), ensuring quality patient care and reimbursement accuracy.

Position Highlights

• Direct the interdisciplinary RAI process in compliance with guidelines for MDS completion and electronic transmission.
• Develop individualized resident Care Plans based on Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license with previous experience in healthcare settings.
• Familiarity with general nursing practices, MDS completion, and RAI/PPS processes.
• Computer proficiency, ideally with AccuMed experience.
• Prior MDS Coordinator experience and BLS/CPR certification.
