Job Description
We are looking for an experienced Licensed Practical Nurse (LPN) to join our HomeHealth team at ChristianaCare. This position offers the opportunity to provide skilled nursing care and treatment to geriatric patients in the community, in alignment with care plans and discharge plans. The successful candidate will be a crucial part of our supportive and innovative HomeHealth environment.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement care plans that are patient-centered and goal-oriented.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
