Job Description
ChristianaCare HomeHealth is seeking experienced Licensed Practical Nurses (LPNs) to join our team. As an LPN in our home care division, you will provide skilled nursing care to geriatric patients in the community, implement care plans, participate in discharge planning, and ensure the highest level of patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.
