Job Description

Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans aligned with resource availability.
- Assist in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
- Available for full-time, part-time, or per diem roles.
- Ability to work day or night shifts, weekends, and holidays as required.
