Job Description
ChristianaCare HomeHealth is seeking Licensed Practical Nurses (LPNs) for full-time, part-time, and per diem positions to provide skilled nursing care to geriatric patients in the community. Join our team and work with a supportive leadership team that offers comprehensive OASIS training and a state-of-the-art Electronic Health Record system.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or General Education Diploma (GED) required.
- Must possess a current DE LPN license or compact state license.
Job Types

Pay
$22.00 - $28.00 per hour
