Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. You will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. This position allows for flexible scheduling options and offers competitive pay based on experience.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
