Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team. As an LPN at ChristianaCare HomeHealth, you will provide skilled nursing care to geriatric patients in the community. This role involves implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
Job Types

Pay
$22.00 - $28.00 per hour
