Job Description
ChristianaCare HomeHealth is looking for dedicated Licensed Practical Nurses (LPNs) to join our team. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement individualized care plans in collaboration with the care team.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
