Job Description

As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning. Responsibilities
- Provide skilled nursing care to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or GED required.
- Current DE LPN license or compact state license.

$22.00 - $28.00 per hour
