Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will have the opportunity to provide skilled nursing care to geriatric patients in the community and help them achieve their health goals.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented, patient-centered, and resource-consistent.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
