Licensed Practical Nurse MDS Coordinator

Job Description

Genesis HealthCare is seeking a Licensed Practical Nurse MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring accurate completion, computer encoding, and electronic state transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Current LPN license in good standing.
• Prior experience in a healthcare setting is required.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer skills, including Windows applications.
• AccuMed experience preferred.
