The Post Acute Unit at UVMHN Helen Porter Rehabilitation & Nursing aims to offer personalized care post-surgery or hospitalization. Our tailored programs focus on patient preparation, support, and education to facilitate a swift return home with maximum independence. We address various conditions including strokes, neurological, orthopedic, respiratory, cardiac, and post-trauma cases.

Routine Therapy services are available Monday to Friday, with the flexibility for six-day therapy support if necessary. Nursing staff extend therapy services during therapist absence, aiding in activities and exercises as directed by therapy plans.
