Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our team at ChristianaCare HomeHealth. As an LPN in our organization, you will provide skilled nursing care to geriatric patients in the community, implementing goal-oriented care plans and participating in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
