Job Description
We are looking for experienced Home Health Licensed Practical Nurses (LPNs) to join ChristianaCare as part of our HomeHealth team. Our organization provides comprehensive Oasis-E training, a state-of-the-art Electronic Health Record system, and supportive leadership to help you succeed in your Home Health career.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.

Work Schedule
- Expected hours: 20 - 40 per week.
- Pay: $22.00 - $28.00 per hour.
- Standard shifts: Day and night shifts, rotating weekends.
- Supplemental pay for differential pay and holiday/overtime shifts.
- Weekly schedule: 5x8, Monday to Friday.
- Work Location: In person.
