Job Description
We are looking for experienced HomeHealth Licensed Practical Nurses (LPNs) to join our dedicated team at ChristianaCare. As a member of our home care division, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning. We offer Full Time, Part Time, and Per Diem positions with competitive hourly rates ranging from $22 to $28.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School diploma or GED required.
- Current DE LPN license or compact state license.
- Experience in geriatrics, home health, pediatrics, or wound care is a plus.

$22.00 - $28.00 per hour
