Job Description
We are currently seeking Licensed Practical Nurses (LPNs) for our Home Health division at ChristianaCare. As an LPN within our team, you will provide skilled nursing care and treatment to geriatric patients in the community. This is an excellent opportunity for LPNs looking to advance their career in home health care.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans in accordance with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
