Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. Our organization provides comprehensive OASIS-E training and utilizes state-of-the-art Electronic Health Records to support our caregivers in delivering high-quality care to our patients.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in coordination with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
