Job Description

We are looking for experienced and compassionate Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, helping them achieve their health goals in a patient-centered environment.

Responsibilities

• Provide skilled nursing care to geriatric patients in the community.
• Implement goal-oriented care plans consistent with available resources.
• Participate in discharge planning for patients.

Requirements

• High School Diploma (HSD) or GED required.
• Must possess a current DE LPN license or compact state license.
