Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. With a focus on providing exceptional care to geriatric patients, you will play a crucial role in implementing care plans and ensuring patient well-being.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Collaborate on goal-oriented care plans that prioritize patient needs.
- Contribute to discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
