Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As a vital part of our organization, you will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community, contributing to their well-being and recovery.
Duties and Responsibilities:
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements:
- High School Diploma (HSD) or GED required.
- Current DE LPN license, or compact state license.
