Genesis Health is seeking a passionate and dedicated LPN (Licensed Practical Nurse) to join our team as an MDS (Minimum Data Set) Coordinator. In this role, you will play a crucial part in the Resident Assessment Process, ensuring high-quality patient care and accurate reimbursement for services provided. Position Highlights

• Lead the interdisciplinary RAI process, adhering to guidelines for MDS completion and electronic transmission.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• LPN license in good standing.
• Prior experience in healthcare facilities.
• Proficiency in general nursing practices and procedures.
• Computer skills, preferably with AccuMed experience.
