Job Description
Genesis Health is seeking a dedicated LPN to join our team as an LPN MDS Coordinator. In this role, you will be responsible for participating in the Resident Assessment Process, completing key items on the Minimum Data Set (MDS), and ensuring appropriate patient care and nursing center reimbursement. As part of our team, you will make a difference in the lives of our residents by delivering quality care and compassion.
Position Highlights
- Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process.
- Develop individualized resident Care Plans and identify resident needs and outcomes.
- Train new team members on clinical content and MDS completion.

*Must be an LPN in good standing and currently licensed by the state.
*Computer skills including experience with windows applications.
*AccuMed experience preferred.
*Previous experience as an MDS Coordinator preferred.
*Demonstrated proficiency in RAI/PPS processes required.
*Current BLS/CPR certificate required.
