Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a skilled Minimal Data Set Coordinator who is a Licensed Practical Nurse (LPN) to join Genesis Health's team. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs). Your work will ensure appropriate patient care and reimbursement for the nursing center.

Position Highlights

• Direct the interdisciplinary RAI process in the nursing center.
• Develop individualized resident Care Plans in compliance with regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing and currently licensed by the state.
• Previous experience in a healthcare facility.
• Knowledge of general nursing practices and regulations.
• Computer skills including Windows applications.
• AccuMed experience preferred.
• Prior experience as an MDS Coordinator is a plus.
• Proficiency in RAI/PPS processes.
• Current BLS/CPR certificate.
