Minimal Data Set Coordinator - Licensed Practical Nurse

Genesis Health is seeking a Licensed Practical Nurse (LPN) to join our team as a Minimal Data Set (MDS) Coordinator. In this role, you will play a crucial part in the Resident Assessment Process by completing MDS items and Care Area Assessments, ensuring proper patient care and nursing center reimbursement. Responsibilities

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process per guidelines, including completing MDS, computer encoding, and electronic state transmission.
• Contribute to personalized resident Care Plans, identifying needs and expected outcomes as per regulations.
• Provide training to team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license and good standing.
• Prior experience in hospital or long-term care settings.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience and knowledge of RAI/PPS processes.
• Current BLS/CPR certification.
