Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a motivated and skilled Minimal Data Set Coordinator who is a Licensed Practical Nurse to join Genesis Health, a leading healthcare services provider. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process, follow completion guidelines, and electronically transmit the MDS.
• Develop individualized resident Care Plans to meet resident needs and outcomes.
• Provide training to the interdisciplinary team on clinical content and MDS completion.

Requirements

• LPN in good standing with current state license.
• Prior experience in a healthcare-related facility.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Proficiency in windows applications and preferably AccuMed experience.
• Experience as an MDS Coordinator and familiarity with RAI/PPS processes.
• Current BLS/CPR certificate.
