Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking to hire a Licensed Practical Nurse for the role of Minimal Data Set Coordinator at Genesis Health. In this position, you will play a key role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs). Your responsibilities will focus on ensuring appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in a healthcare-related facility.
• Proficiency in general nursing practices and MDS completion.
• Computer skills, including familiarity with windows applications.
