Minimal Data Set Coordinator - LPN

Job Description

Genesis Health is seeking a diligent and detail-oriented Minimal Data Set Coordinator who is a licensed practical nurse (LPN). In this role, you will play a crucial part in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion, encoding, and electronic State transmission.
• Contribute to the development of individualized resident Care Plans according to Federal and State regulations.
• Train new team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license and good standing.
• Prior experience in a healthcare setting and familiarity with general nursing practices.
• Proficiency in MDS completion, Care Plans, and RAI/PPS processes.
• Computer literacy, specifically in Windows applications. AccuMed experience is a plus.
• Current BLS/CPR certification.
