Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, assess medical conditions, and evaluate rehabilitation potential.
• Proficiency in EMR documentation is preferred.
• Current CPR Certification.
