Physical Therapist Responsibilities:

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care to improve functional capabilities and overall well-being.
• Document patient information accurately in our EMR system.

Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in EMR documentation.
• CPR Certification.
