Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Proficient in conducting home visit assessments, providing direct patient care, and documenting in EMR systems.
• CPR Certification.
