Physical Therapist Responsibilities:

• Conduct home visit assessments of referred patients to evaluate medical conditions and functional capabilities.
• Provide direct patient care and develop rehabilitation plans.
• Proficiently document patient information in EMR systems.

Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Valid Physical Therapy (PT) license for New York.
• CPR Certification.
• Familiarity with EMR documentation.
