Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to provide direct patient care and assess medical conditions and functional capabilities.
• Proficient in EMR documentation.
• Hold a CPR Certification.
