Job Description

Paradigm Rehab is currently seeking a dedicated Physical Therapist to join our team. As a Physical Therapist, you will play a pivotal role in helping individuals enhance their quality of life through personalized care and treatment.

Responsibilities

• Conduct comprehensive home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care and develop tailored treatment plans to improve functional capabilities.
• Document patient progress accurately using electronic medical records.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold a current, unencumbered Physical Therapy license in New York.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• Maintain CPR certification.
