Physical Therapist Responsibilities:

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and potential for rehabilitation.
• Provide direct patient care and develop treatment plans to improve functional capabilities.
• Utilize EMR documentation for accurate record-keeping.

Requirements:

• Graduated from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• Proficient in CPR.
