Physical Therapist Requirements:

• Graduation from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Proficient in conducting home visit Physical Therapy assessments.
• Experience with EMR documentation is a plus.
• CPR Certification.
