Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, provide direct patient care, and assess medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficiency in EMR documentation preferred.
• CPR Certification required.
